Support Coordinator (Developmental Disabilities)

Definition

This is an entry-level professional case manager position providing support to individuals with a developmental disability for the Phelps/Texas County area. This job is an in office position, and the applicant must reside in the state of Missouri in a surrounding County to Phelps and Texas. Please do not apply if you do not meet this requirement. Applicant must have a thorough understanding of case management and case planning, strong communication skills both written and verbal; a minimum of one years’ experience working with individuals with a developmental disability is preferred. The successful candidate will have the ability to work in a fast-paced social service environment with little oversight, successfully interact with diverse populations, and excel in time management.

Duties and Responsibilities

Duties will include but not be limited to:

  • Manage a caseload of moderate size and complexity providing support to and advocating for eligible individuals with a developmental disability.
  • Excellent verbal and written communication skills with consistent attention to detail and the ability to work collaboratively with others.
  • Strong interpersonal skills are a necessity and the effective candidate will have the ability to work successfully with people from a variety of backgrounds.
  • Develop, in cooperation with the individual, and the rest of the planning team a Person-Centered Plan (individual support plan) for each client. Keep support plans current, modify as needed, and maintain accurate records.
  • Link individuals to services that will allow them to lead a full life and be a valued member of their community. Prepare authorization and funding pages to enable clients to access services.
  • Monitor services to ensure that the service is being implemented by the Provider, review quality of services, and client progress to identified goal.
  • Complete modifications to the Person-Centered Plan (individual support plan) as needed.
  • Attend monthly team meetings concerning new or revised policies and procedures.
  • Receive close supervision from the Support Coordinator Supervisor in addition to other members of the Management Team.
  • Perform other related work as assigned.

Minimum Qualifications.

  • A Bachelor's degree from an accredited college or university with a minimum of 24 earned credit hours in one or a combination of the following: Psychology, Sociology, Special Education, Counseling, or other specialties in human services; AND one of the following:
  • One or more years of professional experience in social work, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, or a closely related area. (24 earned graduate credit hours from an accredited college or university in the specified areas may substitute for the required experience.) OR
  • One or more years of experience in providing direct care to persons with developmental disabilities. (24 earned graduate credit hours from an accredited college or university in the specified areas may substitute for the required experience.) Per the Missouri State Department of Mental Health, Division of Developmental Disabilities, to be qualified as support coordinators (case managers) for targeted case management services, staff must meet at least the minimum experience and training qualifications for the position of Case Manager I with the Division of DD. These qualifications are the same as the minimum required for a Qualified Developmental Disabilities Professional (QDDP). Skills,

Knowledge, and Qualifications

  • Intermediate knowledge of a specific discipline such as social work, psychology, special education, or counseling.
  • Introductory to intermediate knowledge of case management methods, principles, and techniques and knowledge of various developmental disabilities.
  • Basic computer skills and a working knowledge of Microsoft Office with an emphasis on Word and Excel.
  • Must be able and willing to travel to meet clients as needed.
  • This position may require extended periods of sitting, as well as some repetitive movements.

  • Flexes scheduled in response to needs of individual on caseload and/or PCBDD activities, whenever reasonable and possible.
  • Employees are expected to work a 40-hour work week and flex their schedule within the same week as needed. All time worked over 40 hours per week, that can’t be flexed, will be considered overtime and staff will be paid accordingly.
