The Nurse Practitioner is a licensed practitioner who is responsible for managing health problems and coordinating health care for our Hospice & Palliative Program patients per State and Federal rules and regulations and the nursing standards of care. Assess patient needs and diagnose health challenges. The Nurse Practitioner is a skilled clinician who is knowledgeable about a wide range of medical conditions. Will work under the direction of the Medical Director and the Senior Director of Nursing. Able to work in a team setting, ability to interact with the public and other employees positively and promote a cooperative working environment. Ability to work weekends and holidays. Ability to travel to any location throughout our service area.

• Graduate of an accredited Nurse Practitioner Program, Current Advanced Practice Registered Nurse (APRN) Licensure
• Demonstrates commitment to Hospice’s philosophy of care, values, and mission statement
• Provides clinical expertise and works collaboratively within an interdisciplinary care team
• Provides consultation and expertise in the delivery of patient care
• Ability to adapt to a changing environment and handle multiple priorities
• Excellent writing and analytical skills
• Assist Medical Director in conducting hospice patient home visits, palliative care consultation visits and face to face encounter visits.
• Minimum three years’ experience in hospice, home health, general medical surgical, oncology or patient population specific (i.e.: geriatrics, pediatric, etc.).
