Luzerne County is Hiring an Aging Case Aide 2 Near Barre, PA

JOB
This is a paraprofessional position. The employee is responsible to support the Social Services Department staff, including the Care Management Unit, the Intake/Assessment Unit, and the Waiver Unit. The majority of work is with the Care Management Unit. Geographic area covered is Luzerne and Wyoming counties. The employee reports to an Aging Care Management Supervisor 1.
EXAMPLE OF DUTIES
Maintain on-going proficiency in the use of the statewide Department of Aging database known as Well-Sky. Receive Weekly Service Status sheets from the AAA's contracted providers. Reconcile the service status noted on the status sheets with the consumer's response to the actual service status by: Contacting the consumers by telephone to determine status of services and satisfaction with services within 2 weeks after an initial referral has been made to a contracted service provider.Recording responses to the 2-week follow-up telephone calls in a standardized format in the journal section of the AAA Data Base. Contact the assigned Care Manager with any issues or concerns. Field all in-coming telephone calls from consumers who have questions about Aging Programs such as the PA 600 form for the CAO, PACE, Tax/Rent Rebates, LIHEAP, etc. or other programs. Complete applications for the above-named Programs for consumers who come into the AAA as walk-ins. Conduct home visits in the community to those consumers who require applications for these types of applications (PA 600, PACE , Rent/Tax Rebate, LIHEAP, etc.) be done in their own homes. Check the local obituaries each day from the four local newspapers and compare the obituaries to the consumer database. Notify the MIS Department as well as the active Care Manager of any deceased consumers in order that the records are purged and closed. Record in the Well-Sky database any consumer activity that takes place under the umbrella of Person-Centered Counseling. Create these cases in the database and adhere to the mandated follow-up schedule with these consumers. Telephone consumers as assigned for special projects, for example, emergency situations, waiting list status, follow-up telephone calls for unmet needs, Personal Response Units activated, Person-Centered Counseling follow-ups, medical supplies orders, or other reasons as assigned or deemed necessary by the Supervisor. Pick-up or deliver consumer necessities as requested by the Care Managers and Supervisors. For example, emergency medical supplies, emergency food items, paperwork for various applications, etc. Keep track of all activities assigned each month and consumers seen each month for statistical purposes. Xeroxing, Faxing, Scanning, Filing, Mailings, Shredding, Purging, etc, as requested.Attend meetings, in-services, seminars, webinars, etc, as required.Other duties as assigned by the Supervisor.
