Job Overview:

We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a strong background in medical terminology and experience in various healthcare settings, including pediatrics, ICU, acute care, and utilization management.

Responsibilities:

- Coordinate patient care plans and provide support to patients and their families
- Collaborate with healthcare providers to ensure appropriate treatment plans
- Conduct assessments to determine patient needs and develop individualized care plans
- Monitor patient progress and adjust care plans as needed
- Advocate for patients to ensure they receive quality care
- Utilize knowledge of hospital medicine, behavioral health, and critical care in decision-making processes

Experience:

- Previous experience in case management or related healthcare roles
- Familiarity with medical terminology and various medical specialties
- Strong communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team environment
- Experience with inhale, utilization management, and patient advocacy
