Seeking a Full-Time LPN to provide continuous end-of-life care to hospice patients in their homes or our contracted facilities. The LPN will also be asked to perform the duties of an On-Call Runner.

Responsibilities: Under the supervision of the team director will provide continuous care services, routine patient visits, in-home respite care and personal care for patients.

As a Runner the LPN will perform emergency assessment visits as directed by the On-Call Triage RN. Coordinates the patient plan of care with the interdisciplinary team.

Work schedule will be Monday through Friday 8:30am to 5pm.

Qualifications:

• LPN licensed in NY state
• 2 years of LPN experience
• Reliable motor vehicle.
• Ability to work with limited supervision.
• Strong interpersonal skills.
• Collaborative and team-oriented personality.
• Reliable motor vehicle required in order to reach patients on our program.

Job Type: Full-time

Pay: $33.00 per hour

Expected hours: 40 per week
