Social Worker - Psychology/Social Coordinator
Reports to: CEO/President
Direct Reports: None
Shifts: Monday – Friday 9 to 5
Educational Requirements: Bachelor’s Degree or higher in psychology, sociology, special education, social work or other mental health areas. 
Experience: Prior experience working with clients with intellectual disabilities, brain injuries, and/or mental illness.
A Psycho/Social Coordinator is responsible for assessing and working with patients with mental health issues. They work alongside other healthcare professionals to ensure that patients receive appropriate medical care and treatment. In addition to doing assessments, confirming the diagnosis, and treatment of mental health and substance abuse disorders. The Psycho/Social Coordinator shall demonstrate the ability to make clinical judgments in an effective and efficient manner and display critical thinking and performance ability in the coordination of patient care. The Psycho/Social Coordinator works in a variety of settings and provides comprehensive care to individuals/client, families, staff, and communities when applicable.

Job duties & Responsibilities: 
1. Assist with marketing with referral sources such as nursing homes, case managers, care coordinators, and hospitals.
2. Conducts intakes for potential client referrals: gather needed documents and information, for evaluation(s) to build a client file and service plan.
3. Manage quarterly care plan conferences by setting schedules and inviting participants. 
4. Facilitate establishment of mental health services for clients including psychiatrist and counseling, as well as any specialized intervention programs such as I-Start. Work with the mental health professionals to establish a behavior management plan for the clients. 
5. Manage the Restrictive Intervention Plans for HCBS clients. Review quarterly with the Interdisciplinary Team. 
6. Serve as a resource to direct care employees in behavioral management of clients, training staff directly on approaches for client behavioral management. Provide training and guidance on behavior management and de-escalation. Serve as trainer for crisis prevention program.
7. Provide sex education to clients with intellectual disabilities. (Educates families, patients, clients, and communities),
8. Manage conflict between clients with support of direct care staff. 
9. Serve as a resource to direct care staff with navigating community resources and insurances such as SSI, Medicaid, EBT-SNAP, and county assistance.
10. Assess, plan, implement, and evaluate patient care in group home setting, and host home setting.
11. Provides case management, teaches self-care activities, identifies and prioritizes actions based on patient care requirements.
12. Has strong awareness and understanding of suicide prevention.
13. Evaluates signs and symptoms indicating physiologic and psychological changes in the patient's condition.
14. Adheres to policies and procedures and protocols at all times mitigating agency risk.
15. Performs other duties as assigned/necessary.
Minimum Requirements:
  • Must meet all federal, state and local requirements. Passing background checks and drug screens.
  • 2-3 years of work experience in Behavioral Health.
  • Complies with all relevant professional standards of practice.
  • Current CPR certification. Knowledge in clinical skills, medical diagnoses, and treatments.
  • Successful completion of new hire training as applicable to the job.
  • Understand patient confidentiality and HIPAA requirements.
  • Positive and team-oriented attitude.
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills (written and Verbal); proficiency in the English language is required.
