Details:

The On Call Caregiver II position will be responsible for providing care and services for our clients, visiting new patients/clients where they reside, which could include private homes, assisted living facilities, independent living communities and group homes. This position will be for 12 hours on call (6 am to 6 pm) on-call working four(4) mornings/days/evenings on and three(3) mornings/days/evenings off. The position will be guaranteed 10 hours a day.

Qualifications:

Current On Call Caregiver II position be willing to work private pay, VA Clients and State Clients
Admissions and On-call experience also preferred
Cannot refuse work to be paid for on call Caregiver II position must take all assigned shifts given.
Works well within a team environment to collaborate daily with caregivers, social workers, client relations people, volunteers, and supervisors and leadership team.
Work closely with patients/clients on a 1-1 level but get strong support from a patient care team.
Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of our company rules and regulations.
Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits and unscheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
Ability to learn our electronic data records system.
Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area.
