Responsibilities: As a member of our team, your responsibilities would include:

  • Completing the CFC Universal Assessment or PCA Waiver Assessments to help consumers maintain their independence.
  • Educating consumers on available services and supports based on the findings of the Universal Assessment.
  • Coordinating home visits to consumers while adhering to all timelines for work completion and submission.
  • Participating in activities aimed at the successful completion of a Universal Assessment with the consumer’s consent.
  • Developing client plan of services as indicated/requested and educating clients about community supports available.
  • Submitting required documentation to the DSS or contracted entity within mandated timelines.
  • Maintaining a system for tracking assignments and documenting consumer-related activities according to guidelines.
  • Participating in various program committees as assigned and undertaking any other duties as required.

Applicants for this role must possess excellent clinical, interpersonal, and organizational skills, and have experience with community resources and documentation requirements for state-funded programs.

A Bachelor's Degree in Social Work, Human Services, or equivalent is a requirement for this position.
