Job Summary:
We are seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will be responsible for providing support and assistance to individuals and families in need. Your role will involve patient assessment, counseling, data collection, and collaboration with other healthcare professionals.

Responsibilities:
- Conduct patient assessments to identify their needs and develop appropriate care plans
- Provide counseling and support to individuals and families facing various challenges
- Collaborate with healthcare professionals to ensure comprehensive patient care
- Assist with medical documentation and data collection for research purposes
- Advocate for patients' rights and ensure they receive appropriate service.

- Collaborate with special education programs to support students with social-emotional needs
- Offer clinical counseling services to individuals experiencing mental health issues
- Coordinate discharge planning for patients transitioning from hospital to home or other care facilities

Skills:
- Strong patient assessment skills to identify needs and develop care plans
- Proficiency in Children and family counseling.
- Knowledge of ICD-9 coding and medical documentation practices
- Ability to collect and analyze data for research purposes
- Familiarity with special education programs and strategies for supporting students
- Experience in a hospital or clinical setting providing counseling services
- Excellent communication and interpersonal skills for effective collaboration with patients, families, and healthcare professionals
- Empathy, compassion, and the ability to provide emotional support to individuals in distress
