Job Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and providing support to elders and adults with disabilities in need of personal care assistance. This is a rewarding position that requires strong organizational skills, attention to detail, and the ability to work with diverse populations.
Duties:
The responsibilities of the Licensed Practical Nurse include
1. completing and coordinating all applicable clinical assessments and clinical
evaluations;
2. developing and reviewing on an ongoing basis each member's plan of care to ensure their needs are met and updated as needed;
3. provide oversight to direct care aides and caregivers in conjunction with the care manager;
4. reviewing the PCP Summary Form; monitoring the health status of all members to ensure that all needed care is properly delivered;
6. reporting changes in the health status of any member to the member's PCP;
7. reviewing direct care aides and caregiver logs at a minimum of every 30 days;
8. conducting on-site visits with each member at the qualified setting
9. completing a nursing progress note corresponding with each on-site visit or
encounter, or more often as the member's condition warrants;
10. submitting semiannual health-status reports to the member's PCP;
11. planning for and implementing discharges and transition from the programs;
12. conducting an orientation for each caregiver and direct care aides before personal care services begin; and
13. providing ongoing training to caregivers and direct acre aides and on health and aging
14. Assist clients in navigating the healthcare system and accessing appropriate services
15. Collaborate with healthcare professionals, social workers, and other service providers to ensure continuity of care
Experience:
- Previous experience working with , elders and adults with disabilities in a case management role is preferred
- Excellent communication skills, both written and verbal
- Strong organizational skills with the ability to prioritize tasks effectively
- Ability to work independently as well as part of a multidisciplinary team
Job Type: Part-time
Pay: From $32.00 per hour
Expected hours: 20 – 30 per week
