Catalytic Health Partners is Hiring a Discharge Planner/Case Manager Near Peoria, AZ

At Catalytic Health Partners (CHP) our Clinical Liaisons are dedicated to meeting with members in their home focusing on assisting with medical education, attending appointments with members as needed to support their healthcare and social needs, transitional care for both physical and mental health admissions. We do not replace their community providers and clinics, but rather help guide members on how to effectively use their community resources. All our team is remote and, in the field, seeing members in their homes and in facilities as needed to ensure their safe transitions and skill development. Clinical Liaisons work to ensure Catalytic Health Partners and its teams are fully updated on members who are in medical/behavioral facilities and their current and planned treatments. Clinical Liaisons establish communication lines with the facilities to learn the reason for admission, current plan of care, metrics for improvement and planned discharge details. Additionally, Clinical Liaisons communicate with patients ensuring their needs are being taken care of and they have clarity in the current care plan.

What We'll Expect from You and What You'll be Doing:

· Spending your day in healthcare facilities, educating patients about their stay and path forward, which helps to ensure they get the recovery care they need.

· Meeting with patients in their home setting to provide education around behavioral and physical health topics.

· Identifying social determinants of health and providing timely follow-up with resources and plans

· Attending important provider appointments to provide advocation and support.

· Clearly explaining the benefits of our program and how it caters to their unique healthcare needs.

· Gaining patient trust and consent for the program, ensuring they feel cared for and understood.

· Ensuring all transitional care metrics are met, including scheduling appointments with community providers.

· Regularly updating medical records and coordinating with the healthcare team for seamless patient care.

· Taking on additional responsibilities as required for effective patient support.

Area Covered:

This is a field position covering the Northwest Maricopa County area including areas such as Surprise, Peoria, Anthem, Glendale.

Requirements:

· Legally authorized to work in the US.

· Bachelor's degree or higher in a medical, behavioral health or social services area of study required.

· One year or more of experience in healthcare or patient management is preferred (but not required).

· One year or more of experience in discharge planning or supporting patients transitional care is preferred (but not required)

· Strong skills in organization, time management, and adaptability to change.

· Excellent communication skills, both in explaining complex information and in actively listening to patient needs.

· Empathy and teamwork skills are highly valued.
