We are looking for a dedicated a care manager who can be a compassionate communicator with our customers' Korean speaking patient populations. As we grow at this exciting time, we are searching for qualified candidates, who are excited about improving patient engagement and increasing better outcomes. The Care Manager role will be responsible for patient communication and outreach programs, as well as reading and reviewing patients’ vitals data. This position will also have interactions with the client and patients’ clinical staff. This role will work closely with Prevounce’s care team and will report directly to the Director of Care Management.

Responsibilities:

  • Patient outreach and communication and the ability to work effectively with patients and their families
  • Following patient chronic condition care plans and disease state protocols
  • Responsible for identifying barriers or issues that patients may be facing with their care, well being, environment
  • Reading and Monitoring patient data and vital signs
  • Inputting and collecting compliance and reporting information requirements
  • Maintain HIPAA compliance
  • Proactively continue to educate self on improving quality care and professional skills
  • Follow clients’ guidelines and notification preferences on when to be informed about patients and their health and any abnormal results
  • Build bond with the dedicated panel of patients
  • Keeping patient records up-to-date
  • Improve efficiency and reach to interact with more patients

Skills:

  • An understanding of chronic conditions and the ability to follow care plans.
  • Excellent communication skills
  • Bilingual - Korean preferably
  • Experience with Electronic Health Records
  • Great customer and customer/ patient services
  • Patience and empathy with patients and understanding of the nuances of patient engagement
  • Efficient time management
  • Ability to keep organized and time management
  • Willingness to learn new software and technology tools
  • Clear understanding of US healthcare system
  • Understanding of physician practice office’s workflows and operations
  • Collaborative worker and team participation
  • Report and comment notes to patients provider’s team

Education and Requirements:

  • A minimum of 3 years of experience in a healthcare setting, or at least with 1 year(s) experience in case management, care coordination, or remote patient monitoring is preferred.
  • Any training or education in Nursing, Social Work, Hospital Administration, or another directly related clinical field is appreciated
  • Call center communication work with patients work experience is preferred
  • Previous Experience at a physician group or provider is appreciated

Must respect clients and their patients by recognizing their rights; maintaining confidentiality and work tirelessly to earn and keep their trust.
