Position Summary:

As a Personal Care Worker (PCW) at Heart to Heart, you will have the privilege of providing essential non-medical support services to our clients, assisting them with activities of daily living (ADLs) as outlined in their individualized care plans. You will be a companion, a helper, and a source of comfort and support for our clients, ensuring their well-being and happiness.

What You'll Do:

• Provide compassionate, professional care to our clients, focusing on their unique needs and preferences
• Assist clients with activities of daily living, such as:
• Helping with dressing and grooming, ensuring they look and feel their best
• Providing mobility assistance, including gentle transfers and ambulation support, to maintain their independence and safety
• Planning and preparing nutritious meals that cater to their dietary needs and preferences, and providing assistance with eating as needed
• Offering companionship, engaging in meaningful conversations, and participating in activities that bring joy and fulfillment to our clients' lives
• Observe and report any changes in a client's condition to your supervisor, ensuring their health and well-being are always prioritized
• Maintain accurate and timely documentation of all services provided, in accordance with agency guidelines and state regulations
• Adhere to all agency policies, procedures, and applicable state and federal regulations, maintaining the highest standards of care and client confidentiality
• Communicate professionally and effectively with clients, family members, supervisors, and other members of the healthcare team
• Participate in ongoing training and professional development opportunities to enhance your skills and knowledge

What We're Looking For:

• A high school diploma or equivalent
• A genuine passion for helping others and a commitment to providing exceptional care with compassion and respect
• Excellent interpersonal and communication skills
• Reliability, flexibility, and a positive, can-do attitude
• A desire to learn and grow within our organization
• Prior experience in a relevant healthcare setting or completion of a state-approved training program is a plus but not required
