Job Overview:

We are seeking compassionate Personal Care Workers (PCW) to join our team. As a Personal Care Worker, you will provide essential support and assistance to individuals in need of care, ensuring their well-being and comfort.

Reports To: Designated Supervisor

Position Summary:

The Personal Care Worker (PCW) provides non-medical support services to clients in their homes, assisting with activities of daily living (ADLs) and light housekeeping tasks as outlined in the client's individualized care plan.

The PCW works under the supervision of a designated healthcare professional and adheres to all agency policies, procedures, and applicable state and federal regulations.

Qualifications:

  • Education and Training:

High school diploma or equivalent.

Completion of a state-approved training program for personal care services, if required by state regulations.

  • Experience:

Minimum of one year of experience in a relevant healthcare setting or completion of a state-approved training program, as required by state regulations.

  • Language Skills:

Ability to communicate effectively in English, both verbally and in writing.

  • Background Check and Health Requirements:

Successful completion of a criminal background check, as required by state regulations and agency policy.

Compliance with all health requirements, including physical examination and tuberculosis (TB) testing, as mandated by state regulations and agency policy.

Essential Duties and Responsibilities:

  • Client Care:

Provide non-medical personal care services as specified in each client's individualized care plan, which may include assistance with ADLs such as bathing, dressing, grooming, toileting, mobility, and meal preparation, in accordance with agency policies and state regulations.

Accompany clients to medical appointments and treatments when authorized by the agency and in compliance with state regulations.

  • Observation and Reporting:

Observe and report any significant changes in a client's condition to the designated supervisor in a timely manner, following agency policies and procedures.

  • Documentation:

Maintain accurate and timely documentation of all services provided, using agency-approved forms and following agency documentation guidelines and state regulations.

  • Compliance:

Adhere to all agency policies, procedures, and applicable state and federal regulations, including standard precautions, infection control, and client confidentiality (HIPAA).

  • Communication:

Communicate professionally and effectively with clients, family members, supervisors, and other members of the healthcare team to ensure coordinated and appropriate care delivery.

  • Professional Development:

Participate in required in-service education and training programs, as mandated by state regulations and agency policy, to maintain and enhance professional knowledge and skills. • Emergency Procedures:

Follow agency-established emergency procedures in the event of any medical or non-medical situations, and report incidents to the supervisor immediately, in accordance with agency policies and state regulations.

  • Additional Duties:

Perform other duties as assigned by the supervisor, within the scope of the PCW's training and competence, and in compliance with agency policies and state regulations.

Physical Requirements:

  • Lifting and Transferring:

Ability to safely lift, turn, and transfer clients, in accordance with agency policies and state regulations regarding safe patient handling.

  • Mobility:

Ability to safely perform essential job functions, which may include stooping, bending, and standing for extended periods, in accordance with agency policies and state regulations.

  • Travel:

Ability to travel to clients' residences, as required for service delivery and in compliance with agency policies and state regulations.

  • Communication:

Ability to hear and effectively communicate in English, as necessary for the safe and effective performance of job duties.

Work Environment:

  • PCWs primarily work in clients' homes, with potential exposure to various living conditions and environments. The PCW will take necessary precautions to ensure personal safety and the safety of the client, in accordance with agency policies and state regulations.
  • PCWs may be exposed to communicable diseases and other health risks associated with direct client care. The PCW will adhere to all infection control and standard precaution protocols, as established by the agency and mandated by state regulations, including the use of personal protective equipment (PPE) when required.
