BBH is seeking a Crisis Stabilization Coordinator to join our team. The BBH Crisis Stabilization Coordinator focuses on engaging in follow-up services with clients who have recently received a mobile response from the crisis team. This position assists in coordinating access to treatment and timely follow-up. This role requires collaboration with internal and external providers, community stake holders, family systems, and various other entities as needed. Responsibilities:
Attend scheduled services during established business hours to assist with level of care needs, connect with needed resources and on-going assessment of client needs
Administers client-centered services and documents on EHR meeting agency’s documentation guidelines
Creates/reviews individualized plans to mitigate risk, connect with services, follow-up, or refer to stabilization services
Provides short-term follow up services, including psychotherapy, case management, advocacy, and provides referrals to clients that received mobile response services
Provides consultation to internal and external providers regarding access to mobile response services, risk mitigation, triage of care, and other clinical/operational topics
Complies with workflows and data collection requirements for Crisis services
Skillful collaboration with clients, referral sources, hospital staff, police departments, and other community stakeholders
Minimum requirements:
Experience in community-based services program and working with individuals in a mental health setting
Experience with risk assessments and de-escalation (community-based, hospital, or other setting)
Ability to perform successfully in a fast paced environment
Excellent problem solving and communication skills
Preferred Qualifications:
BSW, MSW or Master’s in Counseling
Active Ohio License, LISW and LPCC preferred, or LSW, LPC and MFT
