Lakes Crossing is a Forensic facility, candidates should understand the client population
and that the facility is high-security, similar to a corrections setting. The clients are at Lakes
for competency evaluation or restoration.

Responsibilities:
- Provide case management services to clients, including assessment, planning, and implementation of care plans
- Conduct interviews and assessments to determine client needs and eligibility for services
- Develop and maintain client records, ensuring accurate and up-to-date information
- Collaborate with other professionals, such as social workers and healthcare providers, to coordinate services and resources for clients
- Provide crisis intervention and support to clients in emergency situations
- Assist clients in accessing community resources, such as childcare, workforce development programs, and addiction counseling services
- Advocate for clients' rights and ensure they receive appropriate benefits, such as Medicare or Section 8 housing assistance
- Provide mentoring and guidance to clients to help them achieve their goals

Experience:
- Bachelor's degree in social work or a related field preferred
- Previous experience in case management or a similar role is highly desirable
- Knowledge of crisis management techniques and intervention strategies
- Familiarity with workforce development programs and resources
- Understanding of addiction counseling principles and techniques
- Strong interpersonal skills and the ability to build rapport with diverse populations
- Excellent communication skills, both verbal and written
- Ability to work independently and as part of a team
- Proficiency in using computer systems for documentation and record keeping

Pay: $30.32 - $38.00 per hour
