Monday to Friday : 9:00 am to 1:00 pm ** Flexible hours available
Salary: $20.1923/hour to $22.5961/hour

SHIELDS' mission is to develop, deliver and evaluate culturally sensitive, comprehensive service models that empower and advocate for high-risk families in South Los Angeles.
The Jericho Vocational Services Center (JVSC) utilizes a "one-stop" model for providing vocational and educational training, mentoring, case management, supportive and linkage services to create successful transitions to employment and career pathways. Jericho provides services for any Los Angeles County resident 18 years and older. Additionally, our Jericho program specializes in vocational services tailored for individuals impacted by the criminal justice system.
The Community Health Worker will provide workforce development and career advancement opportunities that promote housing stabilization and family resiliency to homeless or at risk of homeless families in South LA. These Services include but are not limited to: Referral and linkage to employment, Vocational and education services, housing, housing support and other supportive services based on client need.
Primary Duties
  • Assess all incoming referrals to determine program eligibility.
  • Conduct intake and assessment activities with clients including assisting with gathering program eligibility documentation, and completing intake forms.
  • Complete eligibility screening and enroll clients into the Coordinated Entry System (CES).
  • Conduct all required Job Training and workforce development preparation and assessments for referred homeless or at risk of homeless participants.
  • Administer and develop individualized employment plans according to participant's specific needs
  • Assist homeless or at risk of homeless families or individuals with enrollment in one or more of: job training, supplemental workshops, high school or general education classes, Vocational/ Career preparation training (s), and employment placements assistance.
  • Housing navigation and housing support assistance to homeless or at risk of homeless families in South LA.
  • Assist homeless or at risk of homeless families or individuals with obtaining basic needs, such as clothing, food, transportation as well as gaining, restoring, improving and/or maintaining daily independent living skills and community participation and social/leisure.
  • Maintain regular ongoing client contact and tailor the intensity of services provided, including the frequency of face-to-face visits, according to clients level of functioning and acuity of needs.
  • Develop and maintain tracking systems to monitor reporting outcomes and funding requirements according to The Carl & Deutsch Foundation Grant Agreement.
  • Obtain all appropriate consents and authorizations for client's participation in program services and for staff to communicate effectively with partners and other service providers to assist clients with linkages to resources, services and benefits acquisition, coordination of services, client advocacy, and other needed services.
  • Monitor and follow up with clients and service providers to confirm timely completion of referrals and linkages, access to services, and maintenance of services.

Benefits Classification:
Regular, Full-Time Employees (40 hours per week) - eligible for full benefits
Regular, Part-Time Employees (20 or more hours per week with a set schedule) - eligible for benefits at a pro-rated basis
Part-Time Employees less than 20 hours per week - not eligible for benefits
Part-Time Recurrent Hourly Employees - not eligible for benefits

Requirements
•Be proficient in reading, writing in English.
•Possess a working knowledge of conducting interventions for clients with a history of substance abuse, incarceration, mental illness or homelessness.
•Working knowledge of client eligibility screening for entitlement program (DPSS, Voc. Rehab, Cal Works, etc.)
•Working knowledge of compatible service systems including, but not limited to, DPSS, SSI ,etc
•Know and comply with Agency policies and procedures, HIPAA.
•Comfortable working with a diverse population including LGBT, formerly incarcerated and people with co-occurring disorders.
•Present ideas, information, and viewpoints clearly, both verbally and in writing.
•Efficiently use the personal computer to include a variety or word processing software.
•Utilize analysis, experience, and judgment to make decisions.
•Demonstrate commitment to team objectives and SHIELDS For Families philosophies.
•Adapt to changing needs by acquiring new skills and knowledge.
•Current California driver's license and a driving record acceptable to the Agency's insurance carrier.
•Provides services within a culturally sensitive model of practice and is respectful to the community and clients.
