Bachelor's level Case Manager Advocate needed to work with adults with mental illness and substance use disorders from our Paw Paw office. The Case Manager coordinates daily activities for customers in the areas of community living, relationships with significant others, financial management, and vocational/educational involvements; collects background information on customers’ mental health status and treatment; and participates in planning, implementation, and evaluation of customers’ activities. Frequent travel throughout the county in order to provide services, advocate for the customer and to attend meetings. The Case Manager will also provide after-hours on-call on a rotating basis with additional compensation.
Must hold a Bachelor's degree in an appropriate human services field. Requires experience working with individuals who have a serious and persistent mental illness, or individuals who are chemically addicted. Knowledge of and ability to implement community based treatment needed.
Position is full-time (37.5 hours per week).
