JOB SUMMARY

This is a full-time, at will position within the Alzheimer’s and Dementia Services of Northern Indiana, a division of REAL Services, Inc. The Care Specialist is responsible for delivery of programs, education and resource referrals under the direction of the ALZNI Director. The Care Specialist is responsible for communicating and providing fundamental support to the caregivers and families within the service area, networking with other community organizations, attending and planning meetings, events and sponsored efforts that support ALZNI and supported the mission of REAL Services.

ESSENTIAL DUTIES include the following:

· Handle incoming Helpline calls, and prepare/mail pertinent materials to callers, as needed

· Maintain documentation of Helpline and Information and Referral Services

· Assist in the development and facilitation of dementia programs in coordination with the ALZNI Program Manager

· Track number of those served on ongoing basis and report data to the Director

· Coordinate and provide one-on-one care consultation to families, including conducting meeting, linkage to appropriate community resources, problem-solving current/ongoing needs, and appropriate follow-up.

· Participate in projects, grants related to care coaching, and care coordination.

· Represent ALZNI at various health fairs, events, meetings, and programs as requested

· Provide content and recommendations for inclusion in brochures, flyers, social media, and promotional materials for ALZNI programs

· Assist in the procurement and distribution of books, brochures and videos

· Contribute articles and program content for quarterly newsletter as requested

· Make referrals for respite grant application forms

· Other duties as may be assigned and/or requested from time to time in fulfillment of the agency’s mission

EDUCATION AND/OR EXPERIENCE REQUIREMENTS

A Bachelor’s degree (BA or BS) from an accredited four-year college or university and/or applicable years of similar work experience. A background in Social Work is preferred.

· Ability to communicate the agency’s mission is paramount to the success of this position

· Ability to work well in a variety of settings and with a wide diversity of individuals including both professionals and members of the community

· Above average ability to problem solve, self-manage, prioritize, manage projects, meet deadlines, and ensure that tasks are completed in an effective and timely manner.

· Excellent critical thinking skills and ability to exercise independent judgement and decision making on matter of significance

· Ability to be a self-starter, take initiative, and work collaborative in a team setting is critical

· Above average ability in the use of technology and computer programs such as Microsoft Office is relevant to the successful completion of the job

· Displays exemplary verbal and written communication skills

· Outstanding organizational and time management skills

· Ability to maintain confidentiality and discretion

· Approachable with a genuine interest in assisting the agency in meeting its mission

DIFFICULTY OF WORK

Guidelines are generally clear and complete, with judgment required in implementing them. This position is generally an office setting position with occasional in home visitations to clients. This position has the potential to have considerable emotional and empathetic investment in our clients.

WORKING CONDITIONS AND PHYSICAL DEMANDS

While performing the duties of this job, the employee is usually seated, standing, or walking at will in a typical office setting. Very limited physical effort required. Regular and recurring travel to meetings, client homes, and trainings.

· Valid driver’s license, insurance and a dependable vehicle required

CONDITIONS OF EMPLOYMENT

· Successful candidate must submit to a pre-employment drug screening, National Sex Offender Registry check, fingerprinting, a limited criminal history check and a driver check.

· Proof of educational credentials and/or licensing is required at time of employment.

· This position has a 90 day probationary period.

Job Type: Full-time

Pay: $50,000.00 - $52,000.00 per year
