Bedford Stuyvesant Family Health Center is Hiring an Outreach Peer Worker (Bilingual Spanish) Near Brooklyn, NY
The Outreach and Navigation Peer Worker will have overall responsibility for conducting intensive outreach and navigation with HIV-positive patients who have been identified as potentially benefiting from navigation and engagement/re-engagement in medical care and supportive services, with the goal of attaining viral suppression.

- Conduct outreach by re/engage HIV-positive community members in health care.
- Document outreach effort and track locations visited for each patient encounter (tracking documentation provided).
- Escort difficult to engage patients to appointment with PCP, including internal and external providers.
- Attend Case Conference meetings and team meetings monthly.
- Ensure assignments are only obtained by the designed person.
- Submit daily/weekly reports to designed person.
- Other duties as required.
- High School Diploma or GED or equivalent
- Strong interpersonal skills with the ability to work collaboratively
- You must be physically able to walk up 5 flights of stairs to see a potential client.
- You must be able to travel to all five boroughs of New York City by subway and bus.
- Conversational ability in a second language is a plus.
- Knowledge of living with HIV/AIDS a plus.
