Quality Care Coordinator / Population Health Medical Assistant

About the Role
Zenith Family Health is seeking a detail-oriented Quality Care Coordinator / Population Health Medical Assistant to help improve patient outcomes through preventative care, care coordination, and quality measure compliance.
This role focuses on identifying and closing care gaps, coordinating preventative screenings, obtaining outside medical records, following up with patients after ER visits and hospitalizations, supporting medication adherence initiatives, and assisting providers in meeting quality and population health goals.
Responsibilities
- Review charts and identify preventative care gaps
- Coordinate Annual Wellness Visits and preventative screenings
- Monitor quality measures including mammograms, colonoscopies, diabetic eye exams, CMPs, and microalbumin testing
- Obtain and document outside medical records
- Follow up on ER visits and hospital discharges
- Contact patients regarding medication adherence and preventative care
- Schedule follow-up appointments as needed
- Document outreach and quality measures in the EMR
- Collaborate with providers and clinical teams to improve patient outcomes
- Participate in quality and population health meetings
Qualifications
- Medical Assistant certification preferred
- Primary care experience preferred
- Strong organizational and communication skills
- Experience with EMR systems (eCW preferred)
- Ability to manage multiple priorities independently
- Passion for preventative medicine and quality improvement
