Quality Care Coordinator / Population Health Medical Assistant

About the Role: This is a in-depth medical role.
Zenith Family Health is seeking a detail-oriented Medical Quality Care Coordinator / Population Health/ Certified Medical Assistant to help improve patient outcomes through preventative care, care coordination, meeting quality measures and ensuring the clinic stays in compliance. You must have knowledge of HEDIS, Medicare/Medicaid MIPS. Population Health links both MIPS and HEDIS by shifting the focus from treating individual sick patients to managing the overall wellness of an entire group.
This role focuses on identifying and closing care gaps, coordinating preventative screenings, obtaining outside medical records, following up with patients after ER visits and hospitalizations and scheduling follow up visits for annual wellness a , supporting medication adherence initiatives, and assisting providers in meeting quality and population health goals.
Responsibilities
- Review charts and identify preventative care gaps
- Coordinate Annual Wellness Visits and preventative screenings
- Monitor quality measures including mammograms, colonoscopies, diabetic eye exams, CMPs, and microalbumin testing
- Obtain and document outside medical records
- Follow up on ER visits and hospital discharges
- Contact patients regarding medication adherence and preventative care
- Schedule follow-up appointments as needed
- Document outreach and quality measures in the EMR
- Collaborate with providers and clinical teams to improve patient outcomes
- Participate in quality and population health meetings
Qualifications
- Medical Assistant certification preferred
- Primary care experience preferred
- Strong organizational and communication skills
- Experience with EMR systems (eCW preferred)
- Ability to manage multiple priorities independently
- Passion for preventative medicine and quality improvement
