Position Summary
Under the direction of the Senior Director of Healthy Living & Belonging, the Healthy Living Coordinator supports the YMCA’s chronic disease prevention and management efforts by coordinating evidence-based health initiatives, supporting program staff, and providing direct service through instruction of select Evidence-Based Health Initiative (EBHI) programs. This role blends hands-on program delivery with coordination, participant support, and community engagement, ensuring high-quality, welcoming, and impactful Healthy Living programs.
Schedule
- Part-time position ( approximately 20 hours per week)
- Flexible schedule with occasional evenings as required by programming
- Must be at least 18 years of age
- Experience working in health promotion, wellness, community health, or chronic disease programs preferred
- Experience with evidence-based health initiatives (EBHI) or similar structured programs strongly preferred
- Ability to build authentic relationships with people from diverse backgrounds and with a variety of health conditions
- Strong organizational skills with attention to detail and program quality
- Comfortable working with data tracking systems and basic reporting tools (e.g., Daxko, Redcap and similar platforms)
- CPR/AED and First Aid certification required (or willingness to obtain)
- Group Fitness, Health Coaching, or related certification preferred
- Ability to move throughout the YMCA facilities and community sites
- Occasional lifting of program materials ( up to 35 pounds)
- Ability to participate in and support wellness activities as needed
- Coordinate day-to-day operations of Healthy Living and chronic disease prevention programs, including the Diabetes Prevention Program, LIVESTRONG® at the YMCA, blood pressure management, arthritis-friendly movement classes, and other evidence-based initiatives
- Provide direct program delivery by instructing select Evidence-Based Health Initiative (EBHI) classes (training provided), such as Diabetes Prevention, Blood Pressure Management, arthritis-friendly movement classes, and cancer survivorship programs
- Support instructors, lifestyle coaches, and volunteers delivering Healthy Living programs
- Ensure programs are delivered with fidelity to curriculum and YMCA standards
- Maintain accurate program records, attendance, and participant data
- Serve as a welcoming point of contact for participants, supporting engagement, retention, and referrals
- Assist with participant outreach, enrollment, and follow-up
- Support small-group programs and workshops focused on physical activity, social connection, and overall well-being
- Collaborate with YMCA staff across departments to ensure a supportive, inclusive experience for health seekers
- Assist with quality assurance activities and continuous program improvement
- Participate in staff meetings, trainings, and professional development opportunities
- Other duties as assigned
- Serves as an ambassador for Healthy Living programs within the community
- Support referral relationships with healthcare providers, public health partners, and community organizations
- Assist with communication and follow-up related to participant progress when appropriate
