Veterans and Military Services Liaison
The Veterans and Military Services Liaison and Service Coordinator delivers direct case management and care coordination for service members, veterans, National Guard and Reserve members, and military families (SMVF). Core duties include conducting intake assessments, developing treatment plans, coordinating cross-system care with VA and community providers, leading client outreach, and maintaining accurate documentation. Additionally, this role serves as the agency's lead resource by training staff on military culture, providing case consultations, and developing external referral partnerships with veteran-serving organizations.
This position improves access, engagement, and care continuity for SMVF populations across local systems. By building internal staff expertise, streamlining identification workflows, and strengthening interagency partnerships, the role directly enhances WCMHS’s organizational capacity and advances its CCBHC model of accessible, person-centered community care.
Summary of Duties
- Provide strengths-based, person-centered, trauma-informed case management, care coordination, outreach, assessment, service planning, and documentation for SMVF clients and families.
- Coordinate care, service transitions, and handoffs across multiple internal and external systems, including community mental health, primary care, hospitals, VA services, housing providers, and military/veteran-serving organizations.
- Serve as WCMHS's primary internal resource, liaison, and point person for military culture, SMVF identification workflows, service coordination strategies, and complex cases.
- Assist clients and families with navigating system eligibility, benefits applications, rights, and processes while advocating for access to necessary services and addressing care barriers.
- Provide training, education, case consultation, and practical resource tools to WCMHS staff regarding military culture, veteran experiences, common SMVF needs, trauma-informed care, and suicide prevention awareness.
- Represent WCMHS in community coalitions, interagency meetings, and veteran initiatives to build external partnerships, identify service gaps, and improve awareness of agency services.
- Participate in supervision, team meetings, risk response, crisis planning support, data collection, quality improvement efforts, and compliance with HIPAA and 42 CFR Part 2 policies.
Compensation and Benefits
The starting salary range for this position is $60,170 to $63,250 annualized. WCMHS provides a comprehensive benefits package including health, dental, vision, and retirement contribution matching. Summary of Qualifications, Knowledge, and Skills
- Bachelor’s degree in social work, psychology, counseling, human services, nursing, rehabilitation, public health, or a related field required.
- Minimum of 3 years of experience in case management, care coordination, mental health, social services, or related community-based work, with demonstrated experience working with adults, families, and community systems involving complex needs.
- Knowledge of trauma-informed, person-centered, recovery-oriented service delivery and experience coordinating services across mental health, medical, housing, benefits, and community support systems.
- Strong organizational, communication, advocacy, relationship-building, training, consultation, and collaborative problem-solving skills, with the ability to work independently and across teams.
- Commitment to WCMHS's mission, cultural humility, and responsiveness to military and veteran identity, along with strong assessment, planning, follow-up, and documentation practices.
- Ability to manage complex cases involving multiple systems and competing demands while exercising sound judgment, professionalism, and boundaries.
- Valid driver’s license, reliable transportation, and ability to travel within the service area, if required.
