Access Clinician
The Adult Access Clinician/Case Manager is a full-time position providing assessment, case management, and brief psychotherapy to adults in crisis. Core duties include providing immediate follow-up to crisis needs, conducting assessments to determine recommended treatments, delivering brief psychotherapy, providing crisis consultation to treatment providers, and handling discharge planning for adults in acute care facilities. Additionally, the role is responsible for coordinating referrals to longer-term providers, providing case management in collaboration with WCMHS programs and other Washington County agencies, maintaining client case records, and attending weekly individual supervision, group supervision, and staff meetings.
This position impacts the agency and the community by providing rapid, mobile crisis intervention and care coordination for adults in acute distress. By working directly in home and field-based environments and collaborating across Emergency/Urgent Programs, Outpatient, CRT, Vermont psychiatric programs, and local community agencies, the role ensures continuity of care, facilitates safe transitions from acute care facilities, and improves access to appropriate longer-term support systems.
Summary of Responsibilities
- Provide immediate crisis follow-up, assessment, case management, and brief psychotherapy to adults in crisis.
- Coordinate client care, referrals to longer-term service providers, and discharge planning for adults in acute care facilities.
- Deliver crisis consultation to treatment providers and collaborate with WCMHS programs and Washington County agencies.
- Create and participate in treatment team meetings, client advocacy, and behavior management consultations.
- Maintain complete and timely case records for the population served in accordance with agency paperwork deadlines.
- Attend required weekly individual supervision, group supervision, and staff meetings.
Compensation and Benefits
The starting pay range for this position is $63,315 to $66,530 annualized.

Summary of Qualifications and Competencies
- Master’s Degree with relevant experience required.
- Therapeutic experience working in crisis intervention, excellent case management skills, and preferred experience working with adults in home-based settings.
- Demonstrated oral and written clinical interviewing and assessment skills, alongside therapeutic experience in home- or field-based services.
- Excellent organizational and follow-up skills with a strict commitment to completing paperwork on time.
- Ability to be reliable, punctual, flexible, and work effectively both independently and as a team member with WCMHS staff and community partners.
- Must possess a safe vehicle and a clean driving record.
Workplace Conditions
Services are primarily home- or field-based, requiring travel throughout the community with a safe vehicle. This is a regular full-time position with benefits that requires a willing schedule to work until 6:00 PM–8:00 PM 1–2 days per week, with flexibility for the remainder of the schedule.
