Care Manager
The Care Manager in the Community Developmental Services (CDS) division supervises 1 to 8 Service Coordinators while addressing the comprehensive service needs of individuals with developmental and intellectual disabilities and autism in home and community settings. Core duties include providing individual and group supervision, workload management, onboarding, training on electronic medical records (EMR), behavior support plans (BSPs), de-escalation, and conflict resolution. The role also involves directly filling in during service coordinator absences, monitoring quality and performance metrics, responding to and managing crisis situations, delivering direct client care (estimated 8–12 hours per week), providing or arranging client transportation, and completing administrative processes, including weekly electronic record documentation.
This position impacts WCMHS and the community by maintaining continuity of care, service quality, and program sustainability for individuals with developmental and intellectual disabilities and autism. Through regular communication with support teams, guardians, Shared Living Providers, Case Management Organizations, and community providers, the role strengthens collaborative partnerships, supports workforce development, and advances the WCMHS vision of achieving Healthy Neighbors and Healthy Communities through compassionate and inclusive care.
Summary of Duties
- Provide individual and group supervision, onboarding, training, conflict resolution, and performance management for assigned Service Coordinators.
- Deliver direct care (estimated 8–12 hours per week) through community and in-home support, filling in during vacancies or absences to maintain continuity of client care.
- Respond to and manage crisis situations, implement crisis response strategies, and utilize support from Crisis Intervention Teams (CIT) as needed.
- Provide direct client transportation or arrange transportation through community providers to facilitate access to services, appointments, and care.
- Conduct and document staff training on electronic medical records (EMR) systems, crisis intervention, behavior support plans, and regulatory compliance.
- Maintain regular communication with support team members, guardians, Case Management Organizations (CMO), case managers, and community partners.
- Complete timely and accurate electronic medical record documentation by the end of each work week while monitoring quality metrics and value-based payment measures.
Compensation and Benefits
The starting salary range for this position is $56,130 to $59,000 annualized.

Summary of Qualifications and Competencies
- High school completion or equivalent required; Bachelor's degree (BA) preferred.
- Three or more years of experience providing services to individuals with developmental and intellectual disabilities and autism, along with team leadership experience required.
- Valid driver’s license, acceptable driving record, and consistent access to a safe, reliable, insured vehicle.
- Knowledge of Behavioral Support Plans, Individual Support Agreements, Trauma-Informed Care, Harm Reduction, evidence-based service practices, and Adult Protective Services (APS) reporting.
- Strong crisis intervention, de-escalation, conflict navigation, problem-solving, organizational, and electronic medical record documentation skills.
- Proficiency in technology troubleshooting and computer skills, with the ability to maintain professional boundaries, strict confidentiality, and positive regard for clients.
- Commitment to adhering to agency policies, practices, procedures, and WCMHS's mission and vision.
Workplace Conditions
Work takes place in office spaces, program spaces, community settings, and client homes, with work hours potentially including evening and weekend shifts as required. The role requires regular full-time scheduling, constant computer operation, driving, transporting clients who may present challenging behaviors, and potential exposure to bloodborne pathogens or medical emergencies, requiring the use of personal protective equipment (PPE) and adherence to safety and emergency protocols.
