Position Summary
As a Physical Therapist with our Home Healthcare department, you will:
- Evaluate patients and develop individualized care plans
- Provide skilled physical therapy services in patients’ homes
- Educate patients and families on treatment plans and safety
- Collaborate with nurses, occupational therapists, and physicians to ensure optimal care outcomes
- Document and update patient progress according to hospital and regulatory standards
Qualifications
- Current PA license as a Physical Therapist (required)
- Previous experience in home health or acute care preferred
- Strong communication, organization, and assessment skills
- Valid driver’s license and reliable transportation
