Part time position open for a qualified NP in a skilled nursing facility in Green Brook, NJ !

Responsibilities:
- Assessment of all recent admissions and acute issues of both subacute and long-term care patients
- Prevention of needless hospital transfers through early assessment and interventions
- Communication and collaboration with primary care and specialists
Qualifications:
- Previous RN experience - NEW NP GRADS WELCOMED!
- Ability to work as an interdisciplinary team member
- Good critical thinking/ assessment skills and the ability to function in a fast-paced environment
- Excellent written/verbal communication skills and computers
