Key Responsibilities:
Implement Housing First principles to facilitate rapid housing entry and stability for participants.
Provide client-centered case management, building trusting relationships and supporting individualized goal setting.
Connect participants to vital community resources (mental health, substance use, healthcare, employment, benefits, legal aid).
Proactively address housing crises and advocate for participants' needs.
Maintain accurate client records and ensure compliance with HUD regulations and HMIS data standards.
Qualifications:
Bachelor's degree in Social Work, Human Services, Psychology, or related field.
2-3 years of case management experience, preferably with chronically homeless individuals or HUD-funded programs.
Strong understanding of Housing First, trauma-informed care, and harm reduction.
Excellent interpersonal, communication, and organizational skills.
Proficiency in MS Office and HMIS/client management databases.
Valid driver's license and reliable transportation.
Empathy, resilience, and a passion for serving vulnerable populations.
