Upper Valley Services (UVS) is one of Vermont’s designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.
UVS is seeking an experienced Service Coordinator to join our Bradford Team!! You can make a difference in the lives of your fellow Vermonters!
We are offering a generous sign on bonus!! $500 after successfully completing six months and another $500 upon successfully completing one year at Upper Valley Services.
- Service Coordinators are the backbone of our program.
- Upper Valley Services is a person-centered program.
- Service Coordinators meet at least monthly with the shared living provider and participant in their home to support and manage various aspects of the state-required expectations to ensure a healthy environment that meets the participant’s goals and needs.
- Mix of field work and main office in Bradford.
- Background checks are required
- Must have valid driver’s license and insured vehicle

- Thorough initial and ongoing training for Service Coordinators
- M-F 8am-4pm and occasional evening or weekend availability
- College degree in Human Services or related field preferred.
