Hospice Intake Axxess Experience

University Home Care Inc is seeking a dedicated and detail-oriented Hospice Intake specialist with Axxess experience to join our team in Livonia, MI. In this role, you will be responsible for managing intake processes to ensure smooth and efficient admission of hospice patients while maintaining accuracy and compliance with company policies and regulatory standards. This position offers a competitive hourly wage ranging from $18.00 to $25.00.

Responsibilities

• Coordinate and manage hospice patient intake using the Axxess platform.
• Review referral information for completeness and accuracy.
• Communicate effectively with patients, families, and healthcare providers to facilitate service initiation.
• Ensure timely and accurate data entry and documentation in accordance with regulatory guidelines.
• Collaborate with clinical and administrative teams to support smooth transitions of care.
• Maintain confidentiality and compliance with HIPAA and company policies.

Requirements

• Proven experience working with Axxess or similar hospice intake software.
• Strong attention to detail and organizational skills.
• Excellent communication and interpersonal abilities.
• Ability to multitask and prioritize effectively in a fast-paced environment.
• Familiarity with hospice care processes and regulatory standards is preferred.
• High school diploma or equivalent required; additional healthcare-related education is a plus.
