Responsibilities
- Coordinate and manage hospice patient intake using the Axxess platform.
- Review referral information for completeness and accuracy.
- Communicate effectively with patients, families, and healthcare providers to facilitate service initiation.
- Ensure timely and accurate data entry and documentation in accordance with regulatory guidelines.
- Collaborate with clinical and administrative teams to support smooth transitions of care.
- Maintain confidentiality and compliance with HIPAA and company policies.
Requirements
- Proven experience working with Axxess or similar hospice intake software.
