Overview
We are seeking a compassionate and dedicated Experience Hospice Aide to provide exceptional care and support to patients in their final stages of life. The ideal candidate will possess a strong background in patient care, with experience working in hospice, long-term care, or home health environments. The Hospice Aide will collaborate closely with interdisciplinary teams to ensure comfort, dignity, and quality of life for each patient.
Duties
Under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to the patient:
- Bath, care of mouth, skin and hair.
- Bathroom or in using bedpan or commode
- Bed transfers as well as ambulation activities as ordered under the care plan
- Range of motion exercises which the Aide has been taught by appropriate professional personnel.
- Performing such incidental household services as are essential to the patient's well-being at home.
- Preparing meals including special diet.
- Taking patient's temperature, pulse, respirations and blood pressures if assigned by a registered nurse.
- Assist with shopping if the patient has no other resource available.
Provides emotional support and compassionate care to the patient and family unit.
- Demonstrates listening skills.
- Practices patience in voice and manner.
- Offers encouragement when appropriate.
Maintains required competency and skills.
- Document intial and onging every 14 day supervision and evaluation by RN on each case assignement.
- Patricipates in annual performance review.
Reports any changes in patient's conditions or family situation immediately by calling the Hospice office and notifying the RN.
Practices Standard Precautions according to OSHA regulations and policy.
Adheres to all policies and procedural guidelines.
Attends at least twelve (12) hours of in-service education program per year.
Maintains current certification as required by the State.
Maintains current knowledge and practices of infectious disease protocols.
- Skills
- Proven experience working in hospice, long-term care, assisted living, or home health settings.
- Strong knowledge of patient observation techniques and medical documentation practices.
- Familiarity with EMR (Electronic Medical Records) and EHR systems for accurate record-keeping.
- Ability to assist with ADLs efficiently while maintaining patient dignity and comfort.
- Experience caring for individuals with disabilities or developmental challenges is highly desirable.
- Knowledge of dementia care, Alzheimer's care, memory care, and hospice-specific protocols.
- Certification in CPR is required; additional certifications such as medication administration or catheter care are advantageous.
