Essential Duties and Responsibilities:
- Maintain and manage patient electronic health records (EHR) to ensure completeness, accuracy, and timely documentation.
- Review medical records for missing information, signatures, and documentation deficiencies.
- Scan, index, upload, and organize patient documents within the EHR system.
- Process requests for medical records from patients, providers, insurance companies, and authorized third parties.
- Ensure compliance with HIPAA, HRSA, and organizational privacy and security standards.
- Monitor record retention and destruction procedures according to federal and state regulations.
- Assist providers and clinical staff with chart preparation and record retrieval.
- Support quality improvement initiatives through chart audits and documentation reviews.
- Maintain confidentiality and safeguard protected health information (PHI) at all times.
- Participate in training and ongoing process improvement activities
EDUCATION/EXPERIENCE
- High School Diploma or equivalent required
- Associates degree or certification in a Health Information Management program is preferred
- One to two years of experience required, additional experience preferred
- Experience with eCW preferred
