Position Summary
The Care Coordination Administrative Assistant supports operational efficiencies of the Care Coordination program to ensure program components, ensuring program compliance with contract requirements. Responsibilities and Duties
- Engage families who qualify for Care Coordination.
- File/Scan/Attach all documentation/correspondence for current/discharged families in electronic health record and any other system by contractual agreement.
- Maintain Electronic Healthcare Records.
- Collaborate with HR on credentialing process for team members and supports needed data entry.
- Generate compliance reports and distribute to management team on a consistent schedule.
- Assist in referral and case assignment process as needed and know protocol for processing paperwork.
Minimum Qualifications
- A high school diploma or GED and 2 years’ experience in in clerical, business administration or operations, office management or another related field.
- Detail orientated and organized
- Must have strong documentation skills
- Microsoft office proficiency
- Must have a valid driver’s license and be insurable
- Excellent written and verbal communication skills.
- Must be computer literate.
- Must be detailed oriented with the ability to write routine reports and correspondence.

Conditions of Employment:
- Ability to pass a child protective services check with the previous State(s) of U.S. residence for the last five years.
- Ability to pass a tuberculosis screening, work-readiness physical and drug screen prior to employment.
- Ability to pass a Federal and State Fingerprinting Background Check both prior to employment and per the schedule outlined by HR thereafter.
