What are the responsibilities and job description for the Case Manager position at TriSource?
TriSource is looking for a Case Manager to work for an amazing non-profit organization in the Washington, DC area.
This awesome opportunity is full-time, 40 hours/week, Monday-Friday 9am-5pm!
Pay: $24/hour with mileage reimbursement and bonus incentive
*Must be willing to complete Fingerprints and DC clearance (this is an out of pocket expense). *
Qualifications needed:
- Bachelors degree in human services or related field preferred
- Driver's License and Personal Vehcile
- At least 2 years of experience in case management
- Comfortable conducting home visits and transporting clients as needed
- Ability to pass FBI background check
- Strong knowledge of community resources
- Comfrotable working with the homeless population
Responsibilities of the Case Manager will include:
- Assist with the development of daily life skills of the clients
- Ensure that the clients are maintaining proper hygiene, staying drug-free, and attending educational services
- Attend treatment team meetings and provide community/home visits
- Facilitate the individualized service plans for the clients and conducted reviews as needed
- Complete reports and statistical data
