- Providing an outstanding admissions experience for our patients
- Communicating effectively with patients, family members, Case Managers, healthcare providers and co-workers
- Providing education to patients and family members regarding their home healthcare experience
- Reviewing patient charts and ensuring information is clearly communicated to the patients care team
- Serving as a point of contact throughout the admissions process
- Ensuring coordination of care with other team members
- 1 year of Intake experience (Home Health/Hospice)
- Ability to Multi-task with High volume referrals
- Insurance Verification and multi hospital portals (experience is a must)
- Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
- LVN is a plus but not mandatory
- Experience with HCHB EMR
