CNA (Hospice & Home Health)
Shift: PRN
Service Area for Coverage: Southwest, Southeast, Northeast

CNA (Hospice & Home Health)
Shift: PRN
Service Area for Coverage: Southwest, Southeast, Northeast
Responsibilities:
- Provide exceptional care in coordination with the assigned nursing care team.
- Maintain patient records that include any changes in doctor’s orders, diet, or personal living activities.
- Help with patient meals, feeding, and basic activities of daily living.
- Deliver weekly reports to the case manager and review any important changes in patient conditions.
- Educate patients and family members.
- Prepare timely, complete, and accurate clinical notes.
- Communicate effectively with healthcare providers and other care team members.
- CNA license in the state of Texas (required).
- One (1) year hospice experience.
- Able to lift up to fifty (50) pounds.
- Able to work occasional weekends, holidays, evenings.
- Strong communication and people skills.
- Experience with HCHB EMR (preferred but not required)
