What are the responsibilities and job description for the Hospice On Call RN position at Transcend Hospice (2)?
Transcend Hospice & Palliative Care is seeking an RN On Call who will be responsible for ensuring that patient care is coordinated and managed appropriately. A typical schedule will be from Monday 5:00 pm to the following Monday 8:30 am, with 7 days on and 7 days off. The On-Call RN is responsible for ensuring that care and services are delivered per Transcend Hospice & Palliative Care policy and Medicare guidelines. The On-Call RN will triage calls and provide on-call visits, pronouncements, and admissions as needed during scheduled on-call hours throughout our Morris County.
JOB RESPONSIBILITIES:
Making home visits to clients in designated geographic territories.
Performing assigned duties, including administration of medication, wound care, treatments, procedures, and comfort care.
Monitoring clients' conditions and reporting changes to the Clinical Manager.
Following up with, executing, and properly documenting doctors' orders.
Performing client assessments as necessary.
On-call after-hours and weekend visits.
Occasional after-hours admissions.
Ensure that documentation is done promptly in the EMR.
QUALIFICATIONS/SKILLS
A current license as a Registered Nurse (RN) in NJ.
At least two years of recent, verifiable clinical (medical/surgical) experience.
Prior home care and/or hospice experience is strongly preferred but not required.
Solid computer skills: prior experience with electronic medical records (EMR) preferred.
Ability to travel to cases as assigned.
Must have a valid driver’s license and auto insurance.
Work Schedule: Rotating schedule—on-call from Monday at 5:00 pm to Monday at 8:30 am—7 days on and then 7 days off!
