Transcend Hospice & Palliative Care is seeking an experienced and visionary Director of Nursing (DON) to lead our clinical operations and ensure exceptional quality of care across our hospice program. This is a key leadership role responsible for managing nursing services, regulatory compliance, and interdisciplinary team performance. The Hospice Director of Nursing must be able to travel and work in our service areas of Morris County.
JOB RESPONSIBILITIES:
Responsible for providing strategic direction and leadership to hospice nursing staff. Oversee the development and implementation of individualized care plans.Ensure quality and continuity of care for hospice patients and their families.
Ensure compliance with federal, state, and local regulations, including Medicare and Medicaid guidelines.
Monitor and evaluate clinical performance and implement quality improvement initiatives.
Maintain accreditation standards with regulatory bodies such as Joint Commission or CHAP.
Recruit, train, and retain qualified nursing and support staff.
Conduct performance evaluations and provide professional development opportunities.
Ensure proper staffing levels to meet patient needs effectively.
Advocate for patient-centered, compassionate end-of-life care.
Provide guidance and emotional support to patients and families.
Address concerns and complaints promptly to ensure satisfaction with services.
Work closely with physicians, social workers, chaplains, and other team members to coordinate care.
Serve as a liaison between clinical staff and administration.
Communicate effectively with patients, families, and external partners.
Assist in budget preparation and resource allocation.
Monitor expenditures related to patient care and ensure cost-effectiveness.
Identify opportunities for operational improvements and efficiency.
May involve on-call responsibilities.
Ability to handle emotionally challenging situations related to end-of-life care.
QUALIFICATIONS:
A current license as a Registered Nurse (RN) in NJ.At least two years of recent, verifiable clinical (medical/surgical) experience.
Prior home care and/or hospice experience is strongly preferred but not required.
Solid computer skills: prior experience with electronic medical records (EMR) preferred.
Minimum of 3-5 years in hospice or palliative care nursing, with leadership or management experience preferred.
CHPN (Certified Hospice and Palliative Nurse) preferred.
Strong leadership, communication, problem-solving, and organizational skills.
Valid driver’s license, personal vehicle for on-the-job transportation, and evidence of insurance coverage as required by Transcend Hospice & Palliative Care policy required
