Job Description:
We are currently seeking an experienced Community Health Registered Nurse (RN) for a 13-week assignment in Pittsfield, MA. The ideal candidate will have Home Health experience, be proficient with OASIS documentation, and be comfortable providing independent patient care in the home setting.
The Community Health Nurse (CHN) provides, teaches, supervises and documents nursing care of the sick and injured at home.
Position Details:
- Specialty: Community Health RN (Home Health)
- Location: Pittsfield, MA
- Contract Length: 13 Weeks (With high possibility of extension)
- Shift: Days (8:00 AM – 4:30 PM)
- Guaranteed Hours: 40 Hours Per Week
- Weekend Requirement: None (Holiday coverage as needed)
- On-Call Requirement: None
- Floating: Community-based home visits
- Charting System: OASIS
Requirements & Preferences:
- Minimum 1 year of acute care nursing experience required (hospital-based preferred)
- Home Health experience required
- Certified Home Health experience with OASIS proficiency required
- Active Massachusetts RN License required
- BLS (AHA) required
- Current driver's license and auto insurance required
- Must have a reliable personal vehicle
- Knowledge of home care reimbursement and regulatory guidelines required
- Local candidates within 50 miles will not be considered
Skills & Qualifications:
- Strong Home Health and community nursing experience
- Proficiency with OASIS documentation
- Ability to work independently with minimal supervision
- Excellent patient assessment and clinical judgment
- Strong critical thinking and time management skills
- Experience managing an independent caseload
- Excellent communication and patient education skills
- Strong documentation and organizational abilities
- Ability to adapt to changing patient needs and work environments
Pay: $71.00 - $71.38 per hour
