What You'll Do
- Manage a caseload of hospice patients in their homes, assisted living communities, and nursing facilities.
- Develop and coordinate individualized plans of care.
- Perform comprehensive nursing assessments and symptom management.
- Collaborate with physicians and the interdisciplinary hospice team.
- Educate patients and families while providing compassionate end-of-life care.
- Ensure timely, accurate documentation.
- Participate in team conferences and quality improvement initiatives.
Qualifications
- Current Missouri Registered Nurse (RN) license.
- Hospice, home health, or case management experience preferred.
- Strong assessment and critical thinking skills.
- Excellent communication and organizational abilities.
- Valid driver's license, reliable transportation, and current auto insurance.
