Hospice LPN

Three Rivers Hospice in Jefferson City would like you to join our compassionate healthcare team as a Hospice Licensed Practical Nurse (LPN), where your dedication and clinical expertise will make a meaningful difference in patients' lives during their most vulnerable moments. Duties
Deliver comprehensive nursing care to hospice patients, including medication administration, wound care, and symptom management.
Conduct thorough patient assessments to monitor health status, vital signs, and pain levels, adjusting care plans accordingly.
Collaborate with interdisciplinary teams to develop and implement personalized care plans that align with patient wishes and clinical needs.
Educate patients and families on disease progression, medication use, and comfort measures to empower informed decision-making.
.Maintain accurate medical documentation within electronic health record (EHR) systems while adhering to HIPAA regulations for patient privacy.
Provide emotional support and comfort measures, ensuring compassionate care that respects cultural preferences and individual dignity.

Qualifications
Valid LPN license in good standing with the state licensing board.
Experience in hospice or palliative care strongly preferred; experience in critical care, emergency medicine, or long-term care is highly valued.
Knowledge of pain management techniques, symptom control, and medication administration protocols.
Ability to perform physical examinations, vital signs assessment, and basic life support (BLS).
Strong communication skills for effective collaboration with healthcare teams and families.
Demonstrated competence in infection control practices, sterile techniques, and patient safety protocols.
