Three Rivers Hospice is seeking a dedicated and compassionate Registered Nurse (RN) Case Manager to join our team. This role is essential in providing high-quality, patient-centered care while coordinating services for hospice patients and their families.
As an RN Case Manager, you will oversee the plan of care, ensure timely and accurate documentation, and collaborate with an interdisciplinary team to deliver exceptional end-of-life care.
What You’ll Do:
- Manage a caseload of hospice patients across home and facility settings
- Complete comprehensive nursing assessments and develop individualized care plans
- Coordinate care with physicians and the interdisciplinary team (social workers, chaplains, aides, etc.)
- Provide education and support to patients and families
- Ensure timely documentation including visits, recertifications, and care plan updates
- Monitor patient condition and adjust care to maintain comfort and dignity
- Participate in IDG (Interdisciplinary Group) meetings
What We’re Looking For:
- Active RN license in Missouri
- Strong clinical and case management skills
- Excellent time management and organizational ability
- Ability to work independently while collaborating with a team
- Compassionate communication and patient-centered approach
- Reliable transportation, valid driver’s license, and auto insurance
- Hospice or home health experience preferred
