Responsibilities
- Manage and coordinate hospice care for an assigned caseload of patients.
- Perform comprehensive nursing assessments and develop individualized care plans.
- Collaborate with physicians, social workers, chaplains, aides, and other members of the interdisciplinary team.
- Provide education and emotional support to patients and their families.
- Maintain accurate documentation and ensure compliance with hospice regulations.
- Advocate for patients while promoting comfort, dignity, and quality of life.
Qualifications
- Current Missouri Registered Nurse (RN) license.
- Hospice or home health experience preferred but not required.
- Excellent assessment, communication, and organizational skills.
- Compassionate, dependable, and committed to providing exceptional end-of-life care.
- Valid driver's license and reliable transportation.
