Three Rivers Hospice in Mt. Vernon is seeking a dedicated and compassionate RN Case Manager to join our hospice team. This is more than a nursing role - it's an opportunity to bring comfort, dignity, and peace to patients and families during one of life's most meaningful journeys.
As a Hospice RN Case Manager, you will be the primary point of contact for patients and their loved ones, ensuring that care is personalized, coordinated, and delivered with the highest level of compassion. You will manage a caseload of patients in their homes, assisted living facilities, or other care settings, working closely with our Interdisciplinary Team to meet physical, emotional, and spiritual needs.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Provide skilled nursing care, symptom management and emotional support
- Educate patients and families on care processes, medications, and comfort measures
- Coordinate services with physicians, social workers, chaplains, and other team members
- Maintain accurate, timely documentation in compliance with all regulations
Requirements
- Valid RN license with current state registration.
- Strong background in patient assessment, discharge planning, diagnostic evaluation, and case management practices.
- Knowledge of HIPAA regulations, medical terminology, physiology principles, and utilization management processes.
- Ability to handle complex cases involving acute care needs across inpatient and outpatient settings with professionalism and compassion.
- Excellent communication skills for collaborating with Interdisciplinary Teams and educating patients/families effectively.
