Three Rivers Hospice is seeking a compassionate, dedicated Social Worker to join our team serving the Licking, Missouri area. If you are passionate about making a meaningful difference in the lives of patients and families facing a terminal illness, we'd love to hear from you.
As a Hospice Social Worker, you will provide emotional support, counseling, advocacy, and access to community resources for patients and their loved ones. This position offers the opportunity to work in a variety of settings, including hospitals, senior care facilities, and patients' homes, while collaborating with an interdisciplinary hospice team to provide exceptional end-of-life care.
Responsibilities:
- Conduct comprehensive psychosocial assessments to identify patient and family needs.
- Develop and implement individualized care plans and interventions.
- Provide crisis intervention and emotional support during difficult situations.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
- Connect patients and families with community resources and support services.
- Advocate for patients' rights and help families navigate healthcare and social service systems.
- Maintain accurate documentation in accordance with hospice regulations and agency policies.
Qualifications:
- Bachelor's degree in Social Work required; Master of Social Work (MSW) preferred.
- Knowledge of medical terminology and medical documentation practices.
- Excellent verbal and written communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Strong organizational skills and a commitment to maintaining patient confidentiality.
