Hospice MSW (Master of Social Work)

Three Rivers Hospice is currently seeking a compassionate and dedicated Social Worker to join our team in our West Plains location. The ideal candidate must possess a strong interpersonal skills and a commitment to improving the lives of individuals and families in need. This role involves working in various settings, including hospitals, senior care facilities, and client's homes, providing support and resources to clients and their families with a terminal diagnosis.

Duties

• Conduct assessments to identify client needs and develop appropriate intervention plans.
• Provide crisis management support to clients experiencing acute emotional or psychological distress.
• Collaborate with healthcare professionals to ensure comprehensive care for patients in inpatient settings.
• Facilitate access to community resources for home community care and ongoing support.
• Advocate for clients' rights and needs within various service systems, including senior care environments.

Qualifications

• A degree in Social Work is required; Master's degree in Social Work
• Familiarity with medical documentation practices and medical terminology is essential.
• Strong communication skills, both verbal and written, are necessary for effective client interaction.
• Ability to work collaboratively within a multidisciplinary team while maintaining client confidentiality.
• A genuine passion for helping others and a commitment to ethical practice in social work.
