Job Summary
The Community Liaison is responsible for exemplifying the University’s Mission, Vision, and Values by providing outstanding customer service. Reporting to the Program Manager, this position works collaboratively with internal teams—including the Center Director, Program Manager, Project Coordinator, and Lifestyle Change Program staff—as well as external clinical and community partners. Focusing on six designated census tract areas, the Community Liaison is responsible for strengthening relationships with clinical staff and community organizations to support the implementation and coordination of bi-directional referrals through EPIC and/or EPIC CareLink. The role includes assisting with the implementation of a Social Support Services Needs assessment tool in target clinics and promoting awareness of chronic disease prevention and management—particularly diabetes, hypertension, and high cholesterol. The position requires frequent travel, including evenings and weekends, for outreach, recruitment, and health promotion efforts. The Community Liaison must be a strong team player, capable of making moderately complex decisions independently, and committed to advancing health equity through community-based engagement and education.
Key Responsibilities
- Coordinate, develop, implement, and train internal and external clinical staff on utilization of EPIC and/or EPIC CareLink in efforts to promote awareness and increase acquisition of bi-directional referrals.
- Network with clinics, clinical staff, and communities within six designated census tract areas required by CDC promoting awareness and opportunities to reduce complications from diabetes and cardiovascular disease.
- Facilitate opportunities for awareness and health promotion of prevention and management of diabetes and cardiovascular disease specific to hypertension and cholesterol.
- Engage in community outreach within Northeast Texas and six designated census tract areas.
- Assist with implementation of a Social Support Services Needs assessment tool within six designated census tract clinics.
- Promote health education and encourage healthy lifestyles for preventing and managing diabetes and cardiovascular disease.
- Assist with implementing and support for team-based care to enhance self-management of cardiovascular disease within the six designated census tract areas.
- Assist with developing and presenting to clinics and community organizations within the six designated census tract areas.
- Assist Program Manager with quarterly reports.
- Communicate effectively with leadership, team members, and UTT HSC colleagues, project partners, stakeholders, subcontractors, and consultants to achieve outcomes, metrics, and improvement targets.
- Handle administrative functions to promote, coordinate, collaborate, and support the scope of work required to achieve stated goals.
- Handle all phone calls, emails, and in-person interactions cordially and professionally.
- Remain current on chronic disease and behavioral health trends, business practices, and health education and promotion theory.
- Engage in lifelong learning and serves as a role model of professionalism.
- Handle confidential/sensitive information with discretion and adherence to HIPAA rules.
- Engage in team meetings and community outreach efforts as appropriate in meeting project need and scope of practice.
- Other duties as assigned.
Required Qualifications
- Bachelor’s degree in health science, Public Health, Business Administration, or a related field.
- At least three (3) years of experience in networking and/or community relations within health related services or a comparable setting.
- At least three (3) years of hands-on experience using EPIC and/or EPIC CareLink systems.
- A combination of relevant education and experience may be considered in lieu of a degree or specific experience requirements.
Required License
- Valid LVN, MA, or CNA License

Additional Information
The retirement plan for this position is Teacher Retirement System of Texas (TRS), subject to the position being at least 20 hours per week and at least 135 days in length. This position has the option to elect the Optional Retirement Program (ORP) instead of TRS, subject to the position being 40 hours per week and at least 135 days in length.

Clinical Position Requirements
Clinical positions require pre-screening through our Occupational Health department, including a drug test. Employees are required to obtain annual influenza vaccines.
