Key Responsibilities
- Conduct initial and ongoing assessments, including diagnostic evaluation, risk assessment, and level-of-care determination.
- Provide brief therapeutic interventions, crisis management, and patient/family education.
- Maintain a minimum of 4 patient contacts per day (after 6 months).
- Collaborate with primary care providers to develop and implement care plans.
- Monitor patient progress, using standardized screening tools, and communicate updates to the care team.
- Complete professional billing accurately and timely for behavioral health services.
- Assess and manage suicidal, homicidal, or abuse risk; coordinate with crisis teams or authorities as needed.
- Support population health outreach and proactive patient engagement.
- Provide consultation and training to primary care staff on behavioral health and substance use topics.
- Education: Master’s degree in Social Work or Counseling.
- Experience: 1–3 years clinical experience, preferably in primary care.
- Licensure/Certification: Montana LCSW or LCPC license required; BLS required; credentialing with payors within 3 months of hire.
