Salary: $32.26 - $43.68 based on years of experience

A compassionate and dedicated job-seeker is needed for the Hospice Nurse opportunity with our company! As a Hospice Nurse, you will be responsible for overseeing the quality, expert care of your assigned hospice patients.

What You’ll Do:

• Perform patient assessments for comfort and relief of symptoms.
• Establish and implement an individualized plan of care for each patient.
• Assess the need for patient and family education and provide education as needed.
• Obtain appropriate physician orders to achieve symptom management.
• Triage and arrange a home visit schedule, demonstrating efficiency in time and mile management.
• Make referrals to additional IDT members and collaborate with social workers related to their involvement.

What You’ll Bring to Our Team:

• Minimum of two years of nursing experience.
• NJ RN license required; BSN preferred
• Bilingual is a plus!
